Lower blepharoplasty options

There are a number of different options for rejuvenation of the lower eyelid:

Non-surgical

For many patients lower eyelid rejuvenation can be performed through non-surgical methods in combination with skin resurfacing techniques. These non-surgical methods have the advantage of being quick to administer, with minimal swelling or downtime. However, these methods do not address the root cause of the problem and are likely to be more suitable for patients with early or mild age-related changes.

Surgical

In more severe cases, especially where there is skin excess or significant midface descent, lower blepharoplasty surgery may be required. Surgery has the advantage of being able to correct the underlying problems at an anatomical level.

There is also the option of facial fat transfer (fat grafting) which is often performed at the same time to improve the contour of the mid-cheek area.

In most cases, a surgical approach to lower blepharoplasty is associated with a longer recovery but will address the problem more comprehensively with longer-lasting results.

Procedure

During the lower blepharoplasty procedure, the surgeon makes either an internal or external incision just under the eyelashes to remove or reposition fat and excess skin from the lower eyelids. The soft tissue of the midface is usually elevated and suspended to lift the mid-cheek. Fat grafting may be used to correct the hollowing of the orbital rim and midface. The surgery is typically performed under general anaesthesia and while some patients prefer to go home the same day, most patients opt to stay overnight in hospital.

Recovery

Generally lower blepharoplasty is performed as a day or overnight case at a clinic or hospital.

After the procedure it is common to have the following:

  • Some mild discomfort following the procedure, which is usually well controlled with oral medications.
  • Swelling and bruising which persists for 1 to 2 weeks. Overall, in 80% of our patients, the majority of the bruising and swelling should resolve by 2-3 weeks.
  • Sometimes swelling can affect the conjunctiva of the eyes (chemosis) and this can persist for several weeks and sometimes longer.

Most patients usually return to office work after 1 – 2 weeks. Light exercise and physical activities can usually resume after 2 weeks. Heavy sporting activity usually needs 4 – 6 weeks of recovery.

Why do eyebags form?

Ageing changes in both the upper and lower eyelid play a central role in the overall changes of midface ageing. These changes are often multifactorial and several treatment modalities may be combined to give the best possible result. They often lead to:

  • Excess skin or “bags” under the lower lid. A deep groove near the inner part of the lower eyelid – the “tear trough”
  • Outturning and loosening of the lower lid exposing the inner lining of the lower lid (ectropion)
  • Drooping of the midface leads to heavy nasolabial folds (folds on either side of the nose)

This would depend on exactly what you require. In some patients, a “trans-conjunctival” approach may be used which leaves a scar on the inside of the lower eyelid which is not generally a visible area.

In other patients, a “subciliary approach” may be required which will leave a very faint line on the outside of the lower eyelid immediately below your eyelashes. Even though this is a visible scar, in the vast majority of patients this scar will mature and fade over time.
