What is an epicanthoplasty?
Epicanthoplasty is a cosmetic surgery procedure targeting the inner or outer corners of the eyes. It is more commonly performed for the inner corner where the epicanthal skin fold is present in most people of Asian descent, covering some of the structures at the inner corner of the eye.
This surgical procedure alters the shape of the epicanthal skin to enhance the eye’s appearance, often complementing double eyelid surgery.
Medical epicanthoplasty surgery
A “medial epicanthoplasty” may be performed at the time of double eyelid surgery, or separately as an individual procedure. While many techniques have been described, they all aim to reduce or remove the epicanthal fold to create a more parallel double eyelid crease.

What to expect when having an epicanthoplasty in Melbourne?

At your initial consultation, Specialist Plastic Surgeon Dr Frank Lin will provide an individualised assessment of the eyelid position and structure, assess your suitability for surgery, and also take time to understand your concerns and goals. Procedure
Epicanthoplasty is carried out by making small incisions in the epicanthal skin, and then reshaping or removing excess skin. This modifies the shape of the inner corner of the upper eyelid to harmonise the curvature of the eyelid fold. There are small sutures at the inner corner of the eye which are removed a few days later.
Recovery

It is common to have:
- Some mild discomfort following the procedure, which is usually well controlled with oral medications.
- Swelling and bruising which persists for 1 to 2 weeks. Overall, in 80% of our patients, the majority of swelling will be resolved by 2 weeks.
Most patients usually return to office work after one week. Light exercise and physical activities can usually resume after 2 weeks. Heavy sporting activity usually needs 4 – 6 weeks of recovery.

What is the difference between blepharoplasty and epicanthoplasty?
Blepharoplasty is primarily focused on addressing issues with the upper eyelids or lower eyelids, such as the creation or stabilisation of the eyelid fold, or removal of excess skin and fat. In contrast, epicanthoplasty specifically addresses the epicanthal folds at the inner corners of the eyes to complement a double eyelid crease.
Can epicanthoplasty be reversed?
Reversing epicanthoplasty is complex, especially when it involves intricate areas like the double eyelids and the upper eyelid crease. Although it’s not impossible, any reversal would necessitate another surgical procedure with its inherent risks.
How effective is epicanthoplasty surgery on enlarging eyes?
Epicanthoplasty is an operation on the skin of the inner corner of the eyes. It does not affect the eye-opening and does not physically “enlarge” the eyes. It does, however, alter the shape of the upper eyelid fold. When performed in conjunction with Asian double eyelid surgery, it can have the effect of enlarging the contour of the eyes. Some patients find that this contributes to a more “open” appearance to the eyes.
When is the best time to have medical epicanthoplasty surgery?
Not everyone is suitable for this operation and it would depend on the severity of your epicanthal fold. In patients who are suitable the best time for medial epicanthoplasty is at the time of your double eyelid surgery.
The main drawback with this operation is that it potentially leaves a very small visible scar on the inner side of your new double eyelid fold. Compared with a simple double eyelid procedure where the scar is hidden in the depth of the new supratarsal (double eyelid) fold, and is therefore not visible with the eyes open.
Will the medial epicanthoplasty scar be obvious?
This is one of the most common questions asked by patients who are considering the surgery. Dr Lin utilises a technique which places a very small scar within eyelid skin immediately under the new supratarsal (double eyelid) fold. This has the advantage of keeping the scar as small as possible, as well as avoiding the thicker, more oily skin of the nasal dorsum which potentially gives rise to more visible scars.
It is important to realise that all scars have the potential to become thick or unsightly (hypertrophic or keloid scars). Although these are rare on eyelid skin, it is one of the risks that Dr Lin will discuss with you during your consultation.
