Breast reduction surgery, also known as reduction mammoplasty, is a procedure designed to reduce the size and weight of the breasts by removing excess fat, glandular tissue and skin. It also reshapes and lifts the breasts to create a smaller, firmer and more balanced contour that is in proportion with the rest of the body.
During the procedure, breast tissue is removed, reshaped, and the nipple and areola are repositioned to a more youthful height.The areola can also be reduced if needed. Beyond improving shape and symmetry, breast reduction often relieves the back, neck and shoulder pain that comes with carrying heavy breasts.

What to expect
Every patient will undergo a full consultation with their surgeon to ensure they feel informed and confident before proceeding. Your surgeon will discuss your goals for surgery and assess your breast size, weight, shape, symmetry, skin elasticity and any physical discomfort you’re experiencing.
Your surgeon will also evaluate your anatomy, skin quality, breast tissue and overall health to determine the best surgical approach. You will also have the opportunity to ask questions about scarring, recovery, incision patterns and expected outcomes.
Breast reduction surgery is performed under general anaesthesia. The procedure typically takes two to four hours but varies depending on the amount of tissue being removed and the specific technique used.
Breast reduction most commonly uses a vertical (βlollipopβ) incision, which runs around the areola and vertically down to the breast crease. This technique results in shorter scars and often provides a more projected, long-lasting breast shape.
In some cases, a Wise pattern (βanchorβ) incision may be required. This adds a horizontal scar within the breast crease for patients needing a more significant reduction. Your surgeon will recommend the most appropriate incision pattern for your anatomy and goals.
Breast reduction surgery removes excess fat, glandular tissue and skin to create smaller and lighter breasts. The breast tissue is reshaped internally, and the nippleβareola complex is moved to a more natural, youthful position while remaining attached to its blood supply and nerves.
The areola can also be reduced in size if enlarged. In many cases, liposuction is used alongside the main procedure to refine the contour of the chest, particularly around the sides of the breasts and the area in front of the armpits.
For patients with softer, fattier breast tissue, a breast reduction performed using liposuction alone may be possible. Whether this approach is suitable depends on your anatomy, skin quality and the amount of reduction required.
Once the breast tissue has been reshaped and the excess skin removed, the incisions are closed with sutures. Dressings and a supportive bra are applied to minimise swelling and help the breasts settle into their new shape.
Your surgeon will provide personalised advice on how to care for your scars to support optimal healing. This may include silicone gel or tape, moisturising products or gentle massage once healed. Scar appearance continues to improve over 12β18 months.
Recovery
Recovery varies between individuals and depends on the extent of surgery and your bodyβs natural healing response. The recovery process can differ for each patient; however, most will usually be able to go home the day after surgery.
Some downtime is required following breast reduction surgery. Swelling, bruising, tenderness and sensitivity are all normal in the early stages and can be managed with prescribed pain relief. These symptoms gradually improve over the first few weeks.
A supportive surgical or sports bra should be worn day and night for several weeks to reduce swelling and support the new breast shape as the tissue settles.
Most patients can resume driving within one week and return to work within two weeks, depending on comfort levels and the physical demands of their role. Light exercise can typically begin after one month, while strenuous activity, heavy lifting and contact sports should be avoided for at least six weeks to protect healing tissues.
Improvements in breast size, shape and overall comfort are noticeable early, with results continuing to refine as swelling decreases. Scars will soften and fade gradually over 12 to 18 months, becoming less visible with time.
You will receive detailed aftercare instructions, including guidance on caring for your breasts, managing discomfort and recognising any potential concerns. You will also attend scheduled follow-up appointments so your surgeon can monitor your healing and ensure optimal results.

There is no strict upper age limit for breast reduction surgery, and suitability is based more on overall health than age. Many women choose to have the procedure from their late teens onward, provided breast development has stabilised. Breast reductions can also be safely performed later in life, including for women in their 70s and 80s.
Breastfeeding may still be possible after breast reduction surgery, but it cannot be guaranteed. As the procedure reduces and reshapes breast tissue, some women retain the ability to breastfeed while others do not. If you are planning future pregnancies, you may wish to delay the procedure until your family is complete.
Your breasts will appear smaller immediately after surgery, but swelling can temporarily affect their shape and size. Most patients have a good indication of their final size by around six to eight weeks, although the breasts continue to settle and refine over several months. Full healing and final results typically take three to six months.

After two pregnancies, this patient was left with skin laxity and a change in shape to both her breasts and tummy β despite returning to a stable, healthy weight. She was happy with her breast volume but troubled by the drooping and excess skin, and by the loose skin and stretch marks across her abdomen.
We planned a combined approach:
πΉ Mastopexy (breast uplift) β excess skin removed and the nipple repositioned higher to lift and reshape the breast, while keeping her own natural volume, exactly as she wanted. This uses an βanchorβ pattern scar.
πΉ Full abdominoplasty (tummy tuck) β removal of the loose skin and stretch marks below the belly button, with repair of the separated tummy muscles (diastasis recti) that so commonly follow pregnancy.

This patient, a mum of two with a completed family, had thought about augmentation for several years. Her request was simple and one I hear often: a natural, modest increase in volume and improved shape β not a dramatic change. On examination she had reduced upper-pole fullness in both breasts with good underlying symmetry.
Rather than reaching for a number, we planned this properly. Measurements, 2D and 3D imaging morphed to her chosen implant, external sizers tried on in clinic, and a home rice test to let her feel the volume for herself. Her choice: a 275cc round, moderate-profile Mentor micro-textured implant, placed subfascially through an inframammary incision β a plan designed to add soft upper-pole fullness in proportion to her petite frame.
